All of our residents who enter Tolstoy Foundation Rehabilitation and Nursing Center have the same goal – rehabilitate expeditiously and return home.

To achieve this, we are committed to providing the highest level of patient care. Our team of professionals develop customized programs for each resident’s needs at his/her highest level of tolerance. Our dedicated multi-disciplinary team has achieved particular praise for our progressive approach, personal touch and individualized care to rehabilitative therapy.

Restorative therapy is provided 6 days a week to eligible residents – assuring a timely discharge.

We specialize in providing rehabilitation services for the following:

Orthopedic injuries/Joint replacement

Here at Tolstoy we have a outstanding physical therapy and rehabilitation program. We specialize in post orthopedic injuries and joint replacement rehabilitation. Our wonderful physical therapists focus on injuries to the musculoskeletal system. Our program focus on a therapeutic approach to recovery the purpose of which is to correct musculoskeletal limitations and alleviate pain from trauma, illness, or surgery. 

Amputation

For the patients that may have an amputated limb post surgery the faculty here at Tolstoy is well prepared to care for and rehabilitate patients. Getting a patient back to a normal routine is imperative to their recovery. The staff at Tolstoy also specializes in wound care of the amputated location. If a patient has prosthetic limbs the physical therapists on staff are prepared to coordinate a rehab plan that incorporates using the prosthetics regularly so the patient gains a comfortability using the device. 

Stroke

The therapists here at Tolstoy are prepared to care for any patients who may have been diagnosed with a stroke. The programs are designed to help the patient relearn skills lost after having a stroke. Depending on the parts of your brain affected by the stroke, rehabilitation can help with movement, speech, strength and daily living skills. Patients with a stroke can require assistance from several different areas of medicine. The medical staff here will stay in touch with the patients doctors to reach a common goal to help the patient become as independent as possible. 

Balance and Vestibular Disorders

Vestibular disorders can effect some patients for various reasons. This can cause a dizzy feeling or a problem with balance. Our rehabilitation program specializes in contesting this disorder with methods such as repositioning maneuvers, habitation exercises, balance retraining and gaze stability training. With our program specifically focusing on balance and vestibular disorder we can instill a confidence in our patients that when they are faced with side effects from their condition they have the ability to handle themselves.

Respiratory Disorders

With the Covid-19 virus  on the rise our system for caring  for respiratory disorders is  more important then ever. Our pulmonary rehabilitation program is a supervised medical program that helps patients with lung disorders live and breathe better. The therapists here have a series of exercises and breathing techniques to help our patients with chronic and acute pulmonary disorders. 

Cardiac

Cardiac rehabilitation is a very important aspect to our recovery program here at Tolstoy. Our program consists of care that includes exercise, support, counseling and education. This comprehensive program helps patients recover from a heart attack or other heart problem. It offers many benefits that can improve patients health and quality of life.

Neurological Disorders

Our neurological disorder rehabilitation program consists of a state of the art medical approach designed for people with injuries , diseases or disorders of the nervous systems. Neurological rehab can often improve function, reduce symptoms, improve the well-being of the patient and give them back their independence. 

Spinal Cord Injuries

When a patient has a spinal cord injury they require a serious form of rehabilitation. Spinal cord injuries can have various effect on the human body from nerve damage to skeletal damage. Patients require a multidisciplined comprehensive approach from many medical professionals that take each patients needs into consideration and determine the best course of action to help the patient recover. 

Arthritis

Arthritis is the inflammation of the joints where the bones meet. Patients that have arthritis would require a particular focus of rehabilitation  which is on relieving pain and increasing motion in the affected joints. Physical therapy is often recommended to help ease arthritis pain. It can strengthen the muscles that support your damaged joints, help reduce joint stiffness, and improve your range of motion.
