In the assessment phase of our patient care we thoroughly analyze the patients situation and decide what is the best program for them. Our approach is multidimensional assessment where we apply the required areas of medicine to best care for the patient.
At our care plan meetings staff, residents and families talk about life at Tolstoy. We discuss meals, activities, therapies, personal schedule, medical and nursing care, and emotional needs. Residents and families can bring up problems, ask questions, or offer information to help staff provide care for their loved ones.
Our team at Tolstoy are here to support our residents needs even after they have left our care. We have a multi-disciplinary discharge team for our patients that include collaborating efforts with family and health care staff to facilitate a plan for a safe discharge.
At Tolstoy we encourage feedback from our residents to make their experience the best it can be. The residents have an established council that meets once a month to hear and discuss the events, issues and proposals they can bring to our facility to help us improve their experience here.
